By submitting this application for Leave of Absence or Non-Practicing Certificate you acknowledge that Mediation Institute must provide relevant information to the AMDRAS board and facilitate the removal of your name from the National Register of Mediators for the duration of your requested period.
If you are also a FDR Practitioner your Registration as a FDRP may be conditional on having and maintaining national Accreditation as a mediator and/or an approved complaint handling service. You are not covered by complaint handling while on a Leave of Absence or holding a non-practicing certificate. Mediation Institute will inform the AG's department of any Accredited Mediators who are also FDR practitioners who are granted a Leave of Absence or Non-Practicing Certificate.
Our team will review your application and contact you if they require any additional information. Once your application is complete a confirmation will be provided and your current membership subscription will be adjusted.
