You can use this form if you are or were an accredited NMAS / AMDRAS mediator with another accreditation provider and would like to transfer your accreditation to Mediation Institute.

You can use this form if you want to transfer at any of the three stages:

• mid-term
• renewal or less than 2 months since your renewal date
• reinstatement of a lapsed accreditation
